In short
Dark circles have four distinct causes: shadowing from a hollow, genuine pigmentation, thin skin showing underlying vessels, and fluid retention. Filler only addresses the first. Treating the wrong cause is the most common reason under-eye treatment disappoints, and in fluid retention it makes things worse.
What causes it
- Shadowing from hollowing
- A genuine volume deficit at the tear trough casting a shadow. This is the cause filler treats, and the only one it treats.
- Pigmentation
- Actual melanin in the under-eye skin, often genetic and more common in deeper skin tones. Filler does nothing for it. It needs pigmentation-directed treatment and rigorous sun protection.
- Thin skin showing vasculature
- A blue or purple tone from vessels visible through very thin skin. Improving skin thickness and quality helps; adding volume generally does not.
- Fluid retention
- Puffiness that is worse in the morning, after salt or after poor sleep. Filler in this situation makes the area look puffier, because hyaluronic acid attracts water.
Why this area is so often treated badly
Under-eye is the region where the gap between what patients ask for and what their anatomy needs is widest. Someone asks for filler for dark circles, has pigmentation rather than hollowing, gets filler anyway, and is disappointed — or worse, ends up puffy.
A proper assessment establishes which of the four causes is dominant before any treatment is discussed. Most people have a combination, and the plan follows from the proportions.
It is also the most technically demanding area on the face, sitting over vessels where a vascular complication carries a rare risk to vision. Conservative treatment by someone who knows the anatomy is not optional here.
Treatment options
Which of these is appropriate depends on your assessment. Each links to the full detail for that treatment.
Tear trough filler
For genuine hollowing casting a shadow, in carefully selected patients. Conservative volume, correct depth, assessed at two to four weeks.
Rejuran Eye
Improves the quality and thickness of thin, crepey under-eye skin, which helps where vasculature shows through.
Jalupro Young Eye
A skin-quality option formulated for the periorbital area where volume replacement is not indicated.
What treatment cannot do
- Filler treats shadowing from hollowing and nothing else
- In fluid retention, filler makes puffiness worse rather than better
- Pronounced fat prolapse is a surgical problem, not an injectable one
- Sleep, salt and allergy affect this area more than any treatment will
- Pigmentation-driven darkness needs pigment treatment and sun protection, not volume
Frequently asked questions
- Will filler fix my dark circles?
- Only if the cause is a hollow casting a shadow. If it is pigmentation, visible vasculature or fluid retention, filler will not help — and in fluid retention it makes the area look puffier.
- How do I know which type I have?
- An assessment in good lighting, including looking at how the area changes when you tilt your head or press gently. Most people have a combination, and the dominant cause determines the plan.
- Why did my under-eye filler make me puffy?
- Usually too much product, product placed too superficially, or treatment in someone prone to fluid retention. Hyaluronic acid attracts water. It can be dissolved with hyaluronidase.
- Is there a treatment for genetic dark circles?
- Where the cause is pigmentation, it is managed rather than cured, with pigment-directed treatment and consistent sun protection. Expectations should be improvement, not elimination.
